Comparative Influence of Fenofibrate and Vitamin D in Streptozocin-induced Diabetic Nephropathy in Rats

Abstract

Diabetic nephropathy (DN) is a leading cause of end-stage renal disease, marked by the gradual deterioration of kidney structure and function. This study aimed to evaluate the efficacy of fenofibrate (FF) and Vitamin D (Vit D), individually and in combination, against streptozotocin (STZ)-induced DN in Wistar rats.. Thirty adult male rats were divided into five groups: control, STZ-induced DN, STZ+FF, STZ+ Vit D, and STZ+FF+ Vit D. Renal function was assessed by serum creatinine and urea levels, oxidative stress by superoxide dismutase (SOD), malondialdehyde (MDA) and fibrosis by renal transforming growth factor-beta 1 (TGF-β1). In addition, the lipid profile (total cholesterol, triglycerides, LDL, and HDL) and nephrin expression were evaluated. Histopathological analysis of renal tissues was also performed. STZ-treated rats showed significant biochemical and histological deterioration, with elevated serum creatinine, urea, MDA, and TGF-β1 levels, as well as lipid abnormalities, accompanied by reduced SOD activity and downregulated nephrin expression. Treatment with FF and/or Vit D resulted in marked improvement in all measured parameters, including normalization of lipid profiles and restoration of nephrin levels. These findings indicate that FF and Vit D, especially in combination, may offer a promising therapeutic approach to mitigate the progression of DN through antioxidant, antifibrotic, lipid-lowering mechanisms.
