Abstract Background: AHRF raises ICU mortality and causes hypoxemia. Immunity and inflammation are both affected by DMBT1, which is a factor that affects AHRF patients. Aim: Evaluate the expression and prognostic significance of DMBT1 in AHRF patients, as well as the effects of L-tryptophan on the A549 cell line. Methodology : The study comprised 59 patients: 26 with extrapulmonary acute hypoxemic respiratory failure (AHRF), 33 with pulmonary AHRF, and 20 controls. The Biochemistry, Chest, and Critical Care Departments of Assiut University executed the research from January to December 2023. DMBT1 was measured at both molecular and serum levels, and the in vitro treatment of A549 cells with L-tryptophan was evaluated. Results: DMBT1 independently predicts AHRF. Serum and transcriptional levels of DMBT1 were elevated in patients. Mortality attained 90.9% with elevated DMBT1 levels. L-tryptophan improved A549 cell viability and DMBT1 expression in the LPS- A549 cell line. Conclusion: L-tryptophan–DMBT1 axis is a target for future pharmaceutical therapies to improve AHRF survival.
