Predicting Chemotherapy Toxicity in Older Adults with Advanced Solid Tumours Using the CARG Score: A Prospective Study

Abstract

Objective: to evaluate the predictive value of the CARG score for high-grade chemotherapy toxicity in elderly patients with advanced solid tumours. Methods: This prospective observational study included patients aged ≥65 years with advanced solid tumours receiving first-line systemic chemotherapy at Assiut University Hospital (April 2022-2025). Patients were stratified into low-, intermediate-, and high-risk CARG groups. Toxicities were graded using CTCAE v5.0. Associations between CARG groups and high-grade toxicity were assessed. Binary logistic regression identified predictors of high-grade toxicity. Results: Seventy-seven patients were included. Grade ≥ III haematologic and non-haematologic toxicities occurred in 42.9% and 54.5% of patients, respectively. High CARG scores were significantly associated with severe chemotherapy toxicity (p < 0.001), treatment discontinuation (p = 0.022), dose modification (p = 0.045), and treatment delay (p = 0.002). The CARG score was the only independent predictor of high-grade toxicity (OR = 5.1, 95% CI: 1.52–17.02, p = 0.008). Conclusion: The CARG score is a valid predictor of high-grade chemotherapy toxicity in elderly patients with advanced solid tumours.
