الملخص: أهداف البحث: تُعدّ الزائفة الزنجارية ''Pseudomonas aeruginosa'' من الجراثيم الانتهازية المعروفة بمقاومتها الذاتية لعدة مضادات ميكروبية، وقدرتها على اكتساب مقاومة واسعة للأدوية، إضافة إلى امتلاكها عوامل ضراوة متعددة؛ مما يجعل العدوى المكتسبة من المستشفيات صعبة العلاج. ويسهم نظام الإفراز من النمط الثالث وبروتيناته المؤثرة الأربعة، وهي ExoSوExoTوExoUوExoY، في إحداث تأثيرات غازية وسامة للخلايا تؤدي إلى موت خلايا العائل. هدفت الدراسة الحالية إلى تقييم الارتباطات بين جينات exo وخصائص مقاومة المضادات لدى الزائفة الزنجارية ودورها في تطور المرض. طرق البحث: جُمعت 102 عزلة سريرية عشوائية من جنس الزائفة من مختبرات مرضية محلية في كراتشي، باكستان. وشُخِّصت العزلات مبدئيًا بالطرق التقليدية باستخدام وسط عزل الزائفة، ثم أُجري التعرف الكيميائي الحيوي باستخدام شرائط QTS-24 للتأكد من الزائفة الزنجارية، إضافة إلى التأكيد الجزيئي اعتمادًا على جينات الحمض النووي الريبي الريبوسومي 16 S الخاصة بالنوع. وصُنفت العزلات وفقًا لاختبار الحساسية للمضادات الميكروبية، بناءً على إرشادات معهد المعايير السريرية والمخبرية لعام 2022. كما حُددت أنماط الضراوة للعزلات المختارة المقاومة لعدة أدوية باستخدام جينات exo الخاصة بنظام الإفراز من النمط الثالث، وذلك بتقنية تفاعل البوليميراز المتسلسل. النتائج: من بين 102 عزلة، تم التعرف على 44 عزلة بوصفها زائفة زنجارية. ووفقًا لأنماط مقاومة الأدوية، كانت 80.6% (25/31) من العزلات مقاومة لعدة أدوية، بينما كانت 16.1% (6/31) شديدة المقاومة للأدوية، وذلك استنادًا إلى تقييم مؤشر مقاومة المضادات المتعددة. وسُجلت أعلى حساسية للميروبينيم بنسبة 84%، يليه البيبيراسيلين بنسبة 66%، ثم الإرتابينيم بنسبة 64%. وسُجلت أعلى مقاومة للكوليستين بنسبة 100%، حيث عُرّفت المقاومة بوجود تركيز مثبط أدنى لا يقل عن 4 ميكروغرام/مل، كما بلغت مقاومة التيغيسيكلين والأزتريونام 82%. وكانت أكثر جينات exo شيوعًا هي exoS بنسبة 43%، تليها exoT بنسبة 30%، ثم exoU بنسبة 13%، وexoY بنسبة 9%. كما وُجدت ارتباطات ذات دلالة إحصائية بين جينات exoومجموعات أنماط المقاومة وفق مؤشر مقاومة المضادات المتعددة، بقيمة احتمالية أقل من 0.05 وقيمة كرامر تساوي 1.00. الاستنتاجات: كشفت الدراسة عن وجود ارتباطات ذات دلالة إحصائية بين مقاومة المضادات وجينات exo المرتبطة بنظام الإفراز من النمط الثالث، وهو ما يمثل تهديدًا كبيرًا لقطاع الرعاية الصحية. وتبرز الحاجة إلى نظام ترصد منهجي لضمان تعزيز الصحة والعافية. Abstract: Objectives: Pseudomonas aeruginosa is a well-known opportunistic pathogen with intrinsic multidrug resistance, extensive drug resistance, and multiple virulence factors, making hospital-acquired infections therapy resistant. The type 3 secretion system (T3SS) and its four effector proteins, i.e., ExoS, ExoT, ExoU, and ExoY, have invasive and cytotoxic effects that lead to host cell death. The significant correlations of exo genes with the resistance attributes of P. aeruginosa in disease progression were evaluated in the current study. Methods: In total, 102 random clinical strains of Pseudomonas were collected from local pathological settings in Karachi, Pakistan. The strains were conventionally characterized using Pseudomonas isolation agar. Biochemical identification was conducted by using QTS-24 strips to confirm P. aeruginosa, and molecular confirmation based on species-specific 16S rRNA genes. The strains were categorized based on antimicrobial sensitivity testing in accordance with CLSI guidelines. The virulence profiles of the selected multidrug-resistant (MDR) strains were obtained using the specific T3SS exo gene by PCR. Results: Among 102 strains, 44 strains were identified as P. aeruginosa. According to the drug resistance profiles for the P. aeruginosa strains, 80.6% (25/31) were MDR and 16.1% (6/31) were extremely drug resistant (XDR) based on multiple antibiotic resistance index (MARi) evaluations. The highest sensitivity was recorded for meropenem with 84%, followed by piperacillin with 66% and ertapenem with 64%. The maximum resistance was to colistin with 100% antimicrobial susceptibility testing (AST) where the resistance was characterized by a minimum inhibitory concentration value ≥ 4 μg/mL, and tigecycline and aztreonam with 82%. The most prevalent exo genes were exoS at 43%, followed by exoT at 30%, exoU at 13%, and exoY at 9%. Statistically significant correlations were found between exo genes and MARi resistotype groups, with a p-value <0.05 and Carmer's value of 1.00. Conclusions: In this study, significant correlations were detected between drug resistance and T3SS exo genes, which are a severe threat to the healthcare sector, and systemic surveillance is necessary to ensure good health and well-being.
