Compromises In Maxillary Premolar Extraction Orthodontic Finishes: A Retrospective Analysis

Abstract

OBJECTIVES: To evaluate intra-arch and inter-occlusal characteristics in the occlusal schema of patients treated with the extraction of maxillary first premolars. METHODS: Nineteen patients that were treated at the Dental College of Georgia between 2006 and 2024were selected after applying inclusion and exclusion criteria, where all patients had maxillary 1st or2nd bilateral premolar extractions. Various occlusal parameters were then evaluated from the finished records of these treated cases. RESULTS: 11°/o finished class II within the optimal range established by the ABO, and 89% finished “super'' class 11, indicating a deviation greater than 2mm beyond the ideal ABO range. 21°/o finished with premolar rotations, and 79°/o without rotations. 21°/o finished with space remaining in the maxilla, while 79°/o finished without spacing. 58°/o of patients finished with a decreased interincisal angle, while42°/o finished with a normal interincisal angle. 100°/o of patients finished with proclination of lower incisors. CONCLUSIONS: The extraction of maxillary premolars can achieve a functional occlusion, although the majority of patients will have compromises and deviations from ideal in their occlusal scheme.
