Resumen: La insuficiencia cardíaca con fracción de eyección preservada (ICFEp) se ha consolidado como una entidad de alta prevalencia y creciente relevancia clínica, superando en frecuencia a la insuficiencia cardíaca con fracción reducida (ICFEr). Este cambio está relacionado con el envejecimiento de la población y el aumento sostenido de comorbilidades como la hipertensión arterial, la obesidad, la diabetes tipo 2 y la fibrilación auricular.Desde el punto de vista fisiopatológico, la ICFEp se asocia principalmente con disfunción diastólica, remodelado miocárdico y fenómenos inflamatorios crónicos, con un perfil predominante en mujeres y pacientes mayores. Su presentación clínica es inespecífica y heterogénea, lo que, junto a la limitada sensibilidad de biomarcadores tradicionales como NT-proBNP, dificulta su diagnóstico. Para abordar esta complejidad, se han desarrollado herramientas como las puntuaciones HFA-PEFF y H2FPEF, que integran parámetros clínicos, ecocardiográficos y bioquímicos para estandarizar el proceso diagnóstico.La ICFEp requiere actualmente un enfoque diagnóstico riguroso, estratificación fenotípica precisa y estrategias terapéuticas individualizadas que respondan a su complejidad clínica. El objetivo de la presente revisión es establecer una guía práctica para el diagnóstico de ICFEp. Abstract: Heart failure with preserved ejection fraction (ICFEp) has become a highly prevalent entity with increasing clinical relevance, now surpassing heart failure with reduced ejection fraction (HFrEF) in frequency. This change is related to the aging population and a sustained increase in comorbidities such as hypertension, obesity, type 2 diabetes, and atrial fibrillation.From a pathophysiological perspective, ICFEp is primarily associated with diastolic dysfunction, myocardial remodeling, and chronic inflammatory events, with a higher prevalence in women and older patients. Its clinical presentation is nonspecific and heterogeneous, which, together with the limited sensitivity of traditional biomarkers like NT-proBNP, makes its diagnosis challenging. To address this complexity, tools such as the HFA-PEFF and H2FPEF scores have been developed, which integrate clinical, echocardiographic, and biochemical parameters to standardize the diagnostic process.ICFEp currently requires a rigorous diagnostic approach, precise phenotypic stratification, and individualized therapeutic strategies that address its clinical complexity. The objective of this review is to establish a practical guide for the diagnosis of HFpEF.
