Rosai-Dorfman disease (RDD), also known as sinus histiocytosis with massive lymphadenopathy, is a rare proliferative disorder of histiocytes with a generally benign course [1]. Purely cutaneous RDD (C-RDD) is an uncommon form of the disease, characterized by skin lesions without lymphadenopathy or systemic manifestations such as fever, night sweats, and weight loss. This report presents a 15-year-old girl with C-RDD, manifesting as subcutaneous nodules and pigmented macules in the right axilla without lymphadenopathy or constitutional symptoms. Histopathological and immunohistochemical analysis of a biopsy confirmed the diagnosis of RDD, demonstrating emperipolesis. Following the confirmed diagnosis, the patient experienced spontaneous remission of the lesions within three months without any further treatment.
