Introduction Brexpiprazole is a new atypical third-generation antipsychotic. There are few cases described involving overdose.Case We describe a 17-year-old female with a deliberate self-ingestion of 18 mg of brexpiprazole. She developed drowsiness, akathisia, dystonia, and prolonged postural tachycardia (peaking at 161 beats per minute). These symptoms settled after 55 hours of hospital admission with supportive care.Discussion Given the long half-life of brexpiprazole, patients may warrant serial orthostatic vital signs and observation until symptoms and postural hemodynamics improve.
