Malakoplakia is a rare inflammatory condition mainly affecting the genitourinary system. Renal allograft malakoplakia is even rarer, with only a few case reports in the literature. The pathogenesis is linked to chronic infection, like Escherichia coli. The condition is characterized histologically by the presence of pathognomonic Michaelis–Gutmann bodies. Here we report a case of graft kidney malakoplakia presenting 7 years after kidney transplantation, leading to graft loss and initiation of maintenance hemodialysis.
