Intrahepatic portovenous shunt caused by ultraselective conventional transarterial chemoembolization for hepatocellular carcinoma: Report of two cases

Abstract

We report two cases of intrahepatic portovenous shunt (IPVS) caused by conventional transarterial chemoembolization (cTACE) for hepatocellular carcinoma (HCC). Both patients had multiple HCC, and preprocedural images including computed tomography during arterioportography showed no portal vein aneurysms or IPVSs in the liver. All tumors, including two tumors of 8 and 10 mm in diameter on the liver surface, respectively, were treated with ultraselective cTACE using a mixture of iodized oil and chemotherapeutics (epirubicin and mitomycin C) and a gelatin sponge slurry of approximately 0.2-0.5 mm in diameter. In both patients, the first follow-up magnetic resonance imaging performed 2 or 4 months after cTACE demonstrated IPVS in the embolized area near the liver surface. No bilomas were demonstrated in the liver in both patients. Both IPVSs have been gradually enlarging for 11 or 12 years after cTACE without any clinical symptoms.
