Oral health plays a crucial role in the quality of life of oncologic patients, who often present with oral complications arising from both the disease and its treatment. Radiotherapy and chemotherapy can induce significant structural and functional changes in dental tissues. These changes predispose patients to carious lesions, pulp necrosis, and periapical pathologies, which require specific endodontic management. This case report aims to present the clinical and radiographic outcomes of endodontic treatment in an oncologic patient using AH Plus Bioceramic Sealer (AHP-B; Dentsply Sirona, Maruchi, Wonjusi, Gangwondo, South Korea), with emphasis on its antimicrobial and biocompatible properties, as well as its influence on periapical tissue regeneration and patient quality of life. This study was designed as a descriptive, qualitative, and longitudinal case report, conducted at UNINASSAU, Cacoal, Brazil, based on the endodontic treatment of a male oncologic patient aged 35, diagnosed with pulp necrosis and asymptomatic apical periodontitis. The diagnostic and therapeutic procedures took place during the first half of 2024, with follow-up continuing until August 2025. The treated teeth demonstrated periapical lesion regression, absence of clinical symptoms, and radiographic signs of bone regeneration. The use of AHP-B provides effective sealing, biocompatibility, and antimicrobial benefits, with satisfactory clinical outcomes in an immunocompromised context. This case highlights the importance of material selection in endodontic management of oncologic patients. Further longitudinal clinical trials are needed to confirm its long-term efficacy in similar patient populations.
