Ascites in the absence of liver disease poses a diagnostic challenge that requires thorough evaluation. We here report a case of a previously healthy 50-year-old Swiss man who presented with constitutional symptoms and progressive abdominal distension. Extensive diagnostic work-up ruled out hepatic and neoplastic causes. Further invasive procedures including a laparoscopy with histopathological and molecular analyses were required to establish the diagnosis of peritoneal tuberculosis. This case highlights the importance of considering rare etiologies in patients with ascites of unknown origin, even in immunocompetent individuals without known liver disease.
