Association between herniation pit and intra-articular pathology in patients with symptomatic femoroacetabular impingement syndrome

Abstract

Purpose: This study aimed to evaluate whether there was a difference in intra-articular findings between patients who underwent hip arthroscopy for femoroacetabular impingement syndrome (FAIS), with and without a herniation pit (HP). Furthermore, we examined whether HP affected the clinical outcomes of hip arthroscopy. Methods: We retrospectively analyzed consecutive patients who underwent hip arthroscopy at our institution between March 2016 and October 2022. All patients were followed up for at least two years. Patients were classified into two groups based on the presence or absence of HP using radiography, computed tomography (CT), or magnetic resonance imaging (MRI). Cartilage lesions of the acetabulum or femoral head, the extent of labral damage observed during surgery, and clinical outcomes were compared between the two groups. Results: A total of 66 patients were included. Based on the imaging analysis, 30 hips were classified as having HP (HP + group) and 36 hips as having none (HP− group). The HP + group had a significantly higher percentage of high grades of Multicenter Arthroscopy of the Hip Outcomes Research Network (MAHORN) classification for cartilage damage to the acetabulum and Beck classification for labral tear relative to the HP− group (HP+: 30%, HP−: 11%, P = 0.027, HP+: 57%, HP−: 25%, P = 0.028). There were no significant differences in the follow-up period between the two groups in terms of clinical outcomes. Conclusions: Patients with symptomatic FAIS and HP showed significantly more severe damage to the labrum and cartilage on the acetabular side of the joint than those without HP. However, there were no significant differences in clinical outcomes between the two groups with and without HP.
